Short answer

A Significant Data Fiduciary must appoint a Data Protection Officer who represents it, is based in India, is responsible to the Board of Directors or similar governing body, and is the point of contact for grievances (s.10(2)(a)). Rule 9 asks you to publish the DPO's business contact on your website or app and mention it in every response to a Data Principal. This template records the appointment, reporting line and publication; it is a sample, not legal advice.

A sample internal note. Fields ending in a colon are for you to fill in. Adapt the responsibilities to your organisation and have counsel review it.
